Survey: transitions from children's to adults social care
The Department of Health and Social Care has commissioned a survey for professionals who work with young people moving into adult social care, young people who are going through, or preparing for this transition, adults with lived experience of transitioning to adult social care from children’s services and parents, carers, and family members of young people who are going through, preparing for, or have been through this transition. The survey should take ten minutes to complete.
