Survey: Delayed or Difficult Hospital Discharge

Healthwatch England want to hear from people who had a difficult experience leaving hospital in the last 12 months , and from carers, relatives or friends who supported them. This could include:

• A delay leaving hospital
• leaving hospital before you felt ready
• leaving before the right care, support, equipment, medication, transport, housing or follow-up arrangements were in place
• something else made

The survey takes 10-15 minutes to complete.
