Frailty and Dementia Evidence Gathering Exercise  

NHS England and the Department of Health and Social Care have launched an evidence‑gathering exercise to support the development of the Frailty and Dementia Modern Service Framework. They are asking for examples of interventions and innovations that improve quality, outcomes and system performance for people living with frailty and/or dementia, including support for unpaid carers.

Submissions can cover activity across health and adult social care, from early pathway support to complex care, and should align with the 10‑Year Health Plan shifts towards community care, prevention and digital delivery.
