Supporting people with frailty - new report
NHS Confederation have published a new report highlighting the innovative ways that people with frailty can be supported in the community. The report includes examples of good practice from around the country where people have been treated at or near to home. Amongst other things, the report recommends tackling workforce shortages in social care and lifting the barriers to further integration between community providers, primary care and social care.
